Mohsen -Shaban- Mohammadi Kouchaksaraei was pronounced dead on Wednesday September 21 in Qaemshahr in the northern province of Mazandaran. He was shot by a war bullet in the back and under the left shoulder at around 8:30 PM on Wednesday September 21 2022 and died after arriving at the hospital.
“He was shot -in an area- between Taleghani Square and the “Daryaye Noor” shopping centre. ”On the way to the hospital Mohsen who was still alive told his brother that he could not breathe. His brother stopped the car and gave him mouthtomouth resuscitation and cardiac massage… but his lungs were filled with blood…” In the hospital the doctors performed the resuscitation for nearly 15 minutes. “Then they took him to the operating room but Mohsen never returned…”

محسن -شعبان- محمدی کوچک سرایی روز چهارشنبه 30 شهریور در قائم شهر در استان مازندران درگذشت. وی حوالی ساعت 20:30 روز چهارشنبه 30 شهریور 1391 بر اثر اصابت گلوله جنگی از ناحیه پشت و زیر کتف چپ مورد اصابت گلوله جنگی قرار گرفت و پس از ورود به بیمارستان درگذشت.
او بین میدان طالقانی و مرکز خرید دریای نور مورد اصابت گلوله قرار گرفت. در راه بیمارستان محسن که هنوز زنده بود به برادرش گفت که نمی تواند نفس بکشد. برادرش ماشین را متوقف کرد و دهان به دهان او را احیا کرد و ماساژ قلبی داد... اما ریه هایش پر از خون شد...» در بیمارستان پزشکان نزدیک به 15 دقیقه عملیات احیا را انجام دادند. سپس او را به اتاق عمل بردند اما محسن دیگر برنگشت.
