He was executed on Monday, 5 October 2026, in Dastgerd Prison, Isfahan.

Alireza Sepahi, one of the defendants in the Meydan Alikhani case in Isfahan, suffered a heart attack and was transferred to a hospital. It was reported that he was hospitalised at Al-Zahra Hospital in Isfahan. Alireza Sepahi was sentenced to death four times on 8 July 2026 and was executed at Dastgerd Prison in Isfahan on 5 October 2026.

The Meydan Alikhani case was initiated following events on 8 January 2026 in the area between Malekshahr and Kaveh neighbourhoods in Isfahan. The judiciary based the charges on the killing of four officers. In connection with these events, 59 individuals were arrested, with some sentenced to death and others to imprisonment.

The charges in the case included waging war against God, corruption on earth, membership in rebel groups against national security, destruction and arson of public property, and participation in the murder of four officers. The case was heard in the Revolutionary Court of Isfahan, where Sepahi received four death sentences. The death sentences for the defendants were confirmed by the Supreme Court in July 2026 and sent for execution.

The hearings for Alireza and Abolfazl Sepahi were held in prison via video conference. Their family reported restrictions on effective access to legal counsel and the inability to obtain a copy or image of the court's verdict. These restrictions, along with allegations of torture and confessions under duress, were central to objections regarding the judicial process.

The Sepahi family referred to CCTV footage and documents, which they claimed showed that he was elsewhere at the time of the incident. They stated that this evidence was not effectively reviewed in court. Reports related to the case identified the judge who issued Sepahi's four death sentences. Some other defendants also claimed that their statements about his presence at Meydan Alikhani were made under pressure and torture.

علیرضا سپاهی، یکی از متهمان پرونده میدان علیخانی اصفهان، که قرار بود اعدام شود، دچار سکته قلبی شد و به بیمارستان منتقل گردید. گزارش شده است که او در بیمارستان الزهرا در اصفهان بستری بود. علیرضا سپاهی در تاریخ ۱۷ تیر ۱۴۰۵ چهار بار به اعدام محکوم شد و در تاریخ ۱۳ مهر ۱۴۰۵ در زندان دستگرد اصفهان اعدام گردید.

پرونده میدان علیخانی در پی وقایع ۱۸ دی ۱۴۰۴ در محدوده این میدان، میان محله های ملک شهر و کاوه اصفهان، تشکیل شد. قوه قضائیه کشته شدن چهار مأمور را مبنای اتهام های پرونده قرار داد. در ارتباط با این وقایع، ۵۹ نفر بازداشت شدند و گروهی از آنان به اعدام و شماری دیگر به حبس محکوم شدند.

اتهام های مطرح شده در پرونده شامل محاربه، افساد فی الارض، عضویت در گروه های باغی علیه امنیت ملی، تخریب و آتش زدن اموال عمومی و مشارکت در قتل چهار مأمور بود. رسیدگی به پرونده در دادگاه انقلاب اصفهان انجام شد و سپاهی چهار حکم اعدام دریافت کرد. احکام اعدام متهمان در تیر ۱۴۰۵ در دیوان عالی کشور تأیید و برای اجرا ارسال شد.

جلسات رسیدگی به پرونده علیرضا و ابوالفضل سپاهی در زندان و به صورت ویدئوکنفرانس برگزار شد. خانواده آنان از محدودیت دسترسی مؤثر وکیل به پرونده و فراهم نشدن امکان دریافت تصویر یا نسخه ای از رأی دادگاه خبر داده اند. این محدودیت ها، همراه با ادعاهای مربوط به شکنجه و اعترافات تحت فشار، از محورهای اعتراض به روند دادرسی بود.

خانواده سپاهی به تصاویر دوربین های مداربسته و مستنداتی استناد کرده اند که به گفته آنان نشان می دهد او هنگام حادثه در محل دیگری بوده است. آنان می گویند این شواهد در دادگاه به طور مؤثر بررسی نشد. در گزارش های مربوط به پرونده، قاضی صادرکننده چهار حکم اعدام سپاهی معرفی شده است. برخی دیگر از متهمان نیز اظهارات خود درباره حضور او در میدان علیخانی را حاصل فشار و شکنجه دانسته اند.
